Franciscan Missionaries of Our Lady Health System and its covered entity affiliates including Our Lady of the Angels, Our Lady of the Lake, Our Lady of the Lake Ascension, Our Lady of Lourdes, St. Francis Medical Center, St. Dominic Health Services and their subsidiaries does not exclude people or treat them less favorably because of race, color, national origin, age, disability, or sex.

Franciscan Missionaries of Our Lady Health System and its covered entity affiliates and their subsidiaries:

If you believe that Franciscan Missionaries of Our Lady Health System and its covered entity affiliates and their subsidiaries has failed to provide these services or discriminated in another way on the basis of race, color, national origin, age, disability, or sex, you can file a grievance with the Civil Rights Coordinator listed below. You can file a grievance in person or by mail, fax, or email. If you need help filing a grievance, you may call (1800) 315-9566. If you would like to file your grievance in writing you may contact your Civil Rights Coordinator as listed below.
