Notice of Data Breach

This is to notify patients of an incident that involved information for some of our patients. . On April 6, 2026, a team member was fulfilling a request from a physician to provide him with a list of his surgical procedures from 2017 to present. The team member inadvertently sent the list to the wrong email address. The email was sent securely as per policy. We were notified immediately and took steps to recall the email. At this time, we have not received confirmation that the email failed to deliver or that the recipient opened the email. Our Lady of Lourdes Health notified all patients via letter. However, some letters were returned as undeliverable. For this reason, we are posting this notice.
Information in the list included patient name, medical record number, surgeon name, date of procedure, procedure type, and case location.
We do not believe that anything improper has been done with your personal health information; however, we are letting you know about this occurrence so that you can decide what steps you would like to take.
- Review your credit report.
- Review any financial account information.
- Watch your accounts for activity that may not be yours and contact any entity if you notice any improper activity.
