It’s that time of year again! All team members must enroll, re-enroll or make changes to your employee benefits for 2024 during the annual open enrollment period from November 1 – November 15, 2023.*
*There are no exceptions if you miss the deadline for enrollment.
To assist you in the decision-making process, we will be hosting a Virtual Benefits Fair (VBF) that will allow you to learn about all our benefits offerings in an interactive setting. The VBF will go live on October 2nd and remain open throughout the Open Enrollment period. If you visit five booths, you’ll be entered into a drawing to win prizes.

Growing Our Network
FMOLHS has invested time in growing our primary care and specialty network to provide you with convenient access to care. We’ve added more providers, extended hours and virtual visits. All our FMOLHS employed providers are in the EPO network and considered Tier 1 in our PPO network.
Virtual Extended Hours Available in Louisiana
As a team member, you receive priority when seeking care from our providers. Virtual Extended Hours offers video visits outside of normal hours of operation to address problems that don’t require a full physical exam. Team members are able to stay within our FMOLHS Provider network and receive convenient access to care, in the comfort of their home, for free under our EPO plan or for only $5 under our PPO plan. Appointments are available for all ages in Louisiana only with appointments coming soon to Mississippi. Schedule an appointment today using the MyChart app.
We’re working on other ways to expand access to our providers including on-demand and other virtual remote care options. More information will be shared about new offerings throughout 2024 on our TeamTalk app.
Medication Copay Savings and Personal Services Available through RxONE
RxONE is our FMOLHS-owned, in-house pharmacy that offers reduced copays for mail order/ 90-day supply prescriptions and for specialty medications from preferred providers. In addition to discounts, RxONE offers team members personal service through their in-store or curbside delivery options, faster fill times, immunizations and ease of access to our pharmacists. Select an RxONE location as your pharmacy of choice and start taking advantage of these savings and personalized service options for your medications.
Reduced Health Premium Available Through Our Just Premium Program
We recognize that health insurance can be a significant expense, which is why we offer a discounted premium opportunity called, Just Premium. Team members must apply to be considered for the program which is based on total household income. To apply complete this application and submit your 2022 Federal Income Tax Return by November 15.
Hospital Indemnity Insurance
Our new Hospital Indemnity Insurance helps team members pay discretionary expenses when hospitalized including deductibles, copays, medical equipment, daily living expenses, caregiving assistance, etc. at affordable premiums.
Health Plans
Just Premium, our reduced health premium opportunity, has increased maximum household income limits for 2024 across all tiers, expanding access to those eligible for a reduced premium.

HSA & FSA
We have changed HSA, FSA and COBRA providers to Voya Financial. The IRS has increased HSA limits to $4,150* from $3,850 for Individual and $8,300* from $7,750 for Family and FSA contribution limits have been increased to $3,050. (*Combined Employer and Employee maximum contribution)
Vision Plan Moving to EyeMed
We have changed vision providers to EyeMed with a reduction in premiums for Basic Plan and the Buy Up Plan with a new offering for enhanced member health and value benefits at Insight Plus Providers. This gives team members the ability to use both contact lens and frame allowances in the same benefit year and other members-only savings on eyewear, LASIK, hearing aids and more.
Voluntary Life Insurance
We have updated the dependent definition to include “up to age 26, regardless of marital or student status.”
PTO Sellback Enrollment Period
To help team members manage their PTO balances, FMOLHS offers a PTO Sellback program that is available from November 1 – 15, 2023. During this time, you may elect to sell back future PTO accruals The combination of PTO accruals and PTO sellback accruals will not exceed 328 hours. Hours more than that mark will not be paid out.
The FMOLHS customized network (EPO and PPO Tier 1 and 2) is designed to drive care to providers employed by the health system or who are partners of the health system. By using providers and facilities in the FMOLHS customized network, we are helping to preserve the future of our organization and our mission. These providers are aligned with our high quality of care standards and are clinically integrated with our electronic medical record system allowing sharing of data between providers.
Today, services provided by any of the impacted providers are considered in network under the EPO Plan and Tier 2 under the PPO and HDHSA Plans. Beginning January 1, 2024, they will be out of network (no coverage) under the EPO Plan and will be Tier 3 coverage for the PPO/HDHSA Plans. We encourage you to begin reviewing the FMOLHS network if you wish to change providers.

This decision was made with insight and approval from our individual market leadership and with our team members in mind. We are confident in asking team members and their family to choose FMOLHS providers, which benefits the ministry, team members and their families.
