There are a lot of choices to make throughout your career with us, both for your professional and personal life. The education lab is meant to be an educational resources to aide you in your decision-making process around Total Rewards.   • Where to Go For Care

    Think of your Primary Care Physician as your go-to for your health care needs. Your PCP plays an important role in your healthcare journey. Ultimately, building a relationship with your PCP and going to them regularly for illnesses, yearly checkups and screenings can lead to better health outcomes and a higher level of satisfaction with care.

    Road Map to Care

    • First Stop - Primary Care Physician (PCP) Having one doctor who knows your overall health history and can better guide you as you navigate certain health risks leads to better patient experience.
    • Second Stop - Urgent Care or Video Visits – If you come down with a minor illness or infection and your PCP is unavailable for an in-person visit because you are traveling, it’s the weekend or it’s later in the evening, Video Visits and Urgent Care are good options to consider.
    • Third Stop - Emergency Room – There are several medical conditions that are considered emergencies because treatment is only available in a hospital setting. Such as chest pains, serious burns, head injuries, seizures and open fractures.

  • Dependent Verification

    You must submit dependent verification documents by the deadline for any newly enrolled and previously unverified dependents enrolled in the Health, Dental or Vision plans or their coverage will be terminated.

    New Hires/Newly Eligible Enrollment and Dependent Verification Deadline: Complete your benefit enrollment and submit complete dependent verification documentation within 30 calendar days of your eligibility date. (e.g., New hire date: May 1, Document deadline: May 30).

    Open Enrollment and Dependent Verification Deadline: Complete your benefit enrollment and submit complete dependent verification documentation by November 15. 

    Dependent Documents Required/Accepted
    Natural Child Birth Certificate; For newborns, birth letter from hospital
    Step Child Birth Certificate and verification of current marriage between our employee and natural parent (see spouse verification below)
    Adopted Child/Child Placed for Adoption Adoption Certificate/placement letter from court or adoption agency for pending adoptions
    Foster Child Proof of Legal Custody, such as a court order
    Child covered under a QMCSO Copy of QMCSO
    Grandchild Proof of Legal Custody and copy of tax return that identifies grandchild as a dependent
    Spouse Marriage Certificate; AND current or previous year tax return OR proof of current joint ownership (such as a joint mortgage, joint rental agreement, joint bank account, etc.)

    FMOL Health reserves the right to audit dependent verification documents at any time.

    After enrollment is complete in your Oracle Self Service, upload the required documents to your 'To Do' list. For more information on Dependent Verification, please reference these frequently asked questions.

    Effective January 1, 2026, Capital Rx will be the new administrator for FMOL Health prescription plans, replacing Express Scripts. Similar to how the medical plan covers doctor visits, the prescription plan includes coverage for eligible prescribed medications.

    The health system partners with SupportLinc to provide EAP services to team members and their families. Services include:

    • In-the-moment support
    • Short-term counseling
    • Coaching
    • Access to Work-life specialists – receive expert consultations for financial and legal issues. Specialists can also provide referrals for everyday needs, such as child or elder care, pet care, home improvement or auto repair.
